Achalasia – Three year after POEM surgery

This week I had my third check-up. The doctor had a look to my esophagus and how looks the stomach. As far as time passed everything looks quite good. I also feel well and do not feel anything disturbing. I still take Esomep 40mg per day, in the morning before breakfast. The signs of acid in the lower part of the esophagus are not growing, everything stays stable.

I’m very happy how everything worked for me and I hope all of you having achalasia and you also did the POEM procedure are in the same way happy with your decision.

So until next year with a new report….

  1. I am concerned about having the POEM procedure. The one Doctor I spoke with their office, I wasn’t allowed to meet the doctor, or have a consult, or discuss any of my critical illnesses, the receptionist wanted me to just set up a surgury date sight unseen. This seems odd to me, I have never had a surgury without meeting or even talking to a doctor prior.
I have been diagnosed with type 3 vigorous Achalasia, dysphasia, rectocele, Cardiospasms, bradycardia, Duodenitis, esophagitis. And then a list of chronic illnesses 3 pages long. I have lost over 50 pounds and am at a complete loss of what to do. I don’t know where to turn to or who to call or if I should just live with this. I am not prepared to go into surgury with a doctor I don’t know and he will not meet with patients for follow ups after the procedure. I am just reaching out for some sort of help or guidance or something……….
    Thank you

    • Hi William,
I would not do any surgery before not meeting with the doctor in charge. If you have any chance to choose, so just search another doctor with very good experience in doing POEM procedure. Try to contact Prof. Dr. Inoue from Japan, maybe he can recommend you somebody. Tell him where you live and I’m sure he can give you some advise.
      Good luck and best regards,
      Roger

      • Thank you Roger I appreciate your help. I am new to this site how do I contact Prof. Inoue?
Again, many thanks and good health to you

  2. Nov 2015 I had the POEM procedure done and 6 months later it has been found that it did not work….
    Who else has found this
When lying flat reflux causes the liquid to enter my voice box and I choke

    • I had POEM done for
Vigorous Achalasia Type 3 two years ago I have lost 72 lbs and am extremely
Underweight, I was told I will have
      dysphasia, Hypotension, cough and never
Speak normal again , be unable
To breathe properly for the test of my life
All the issues are below and getting worse
(These are just some of the problems)
There are 3 types what do you have?
I’m so sorry for what you are going
      Through. I hope u didn’t go to a chop
Shop but Mayo or Stanford or the like. The chop shop wanted to send me home right
After surgery and had never dealt with Type 3
Before. I ended up being in the hospital for 4 days they wanted me there longer. I wish the best for you and hope you the best recovery

      • Hi Mr Williams

I had the P.O.E.M procedure done also and it has not worked either…..

Waikato Hospital NZ Brought Dr Frockem out from Holland to operated on 6 of us….The 1st one was a failure and the last one , Me was a failure after 4hr 15 minutes…We here in New Zealand are also trapped from financial restrains put on by our NZ Govt…..I cannot afford to go private and the operation would have to be done overseas which would cost in excess of $70.000 NZ which I cannot afford…
Our NZ Govt has put financial restraints on our health care and want us to go to private insurance which we elderly cannot afford to pay….6k out of our pension of 14k after tax is not on

My pain level before the November 15 2015 operation was 7/10 plus and now after the failure of the operation is back up to 6 or 7/10 which I am on morphine of 100 mls plus a day again..

It now appears that here in NZ any way have no choice if we have the money to go overseas were the success rate is a lot higher..
It Took over 2 years to diagnose the complaint and in that 2 years the acid from my stomach (which is more potent than battery acid) has burnt by oseophegus / gullet which
        is now completely dead..

I wish you well Mr Williams as I know its is a terrible thing to go through

  3. Dear roger,

I’ve been reading your hole case and its a great thing to have these positive results about POEM.

I had heller’s myotomy on 2013 and didn’t improved only 30%, Till 2016 I have contacted Prof.Inoue to get an appointment for POEM and finally I’m here in Zurich and had done POEM last week with Prof.Inoue and Prof.Seewald.

After the surgery the did for me endosocpy to check everything is ok and no leaking in the esophagus and they told me everything is ok. First week go with liquids and after your first week you can eat anything.

Today is my 8th day I tried my first meal pasta and eggs (wanted to try soft food) and I felt a bit improvement in swollowing but only a little pain from the clips I can say. But my question is when will I experience the best swollowing with eating anything? Is it after 1 week or 1 month or when exactly? Coz I read your post that your able to eat whithout drinking but for me I ate today and had to drink water with my meal. So I assumed if i ate pizza or mcdonalds i will need a bottle of water for sure.

    Thanks for your posts.

    • Hi, I am glad to hear that you had almost a positive experience. You need to have patience, in my case I was able to eat totally normally after one month. I am not feeling that I need water for eating, everything goes down smooth. But one anyway is drinking while eating, so it should be actually no problem. Good luck.

    • I’m so happy your recovery went so well.
I had POEM 2 years ago and still cannot
Eat solid foods. I get very very ill if I try to eat
Anything but “mush”. I have also lost 70
Lbs. this is just to name a couple issues.
My health is declining rapidly and I will have Chronic Vigorous Achalasia Type 3 for
The rest of my life. Cannot eat, speak,
And other issues I’m sure no one wants the
      Gorry details
Best to you. I’m very happy to hear
      Your recovery went well!

  4. I had the P.O.E.M procedure done in November 15…the first 3/4 months were good but since then chest pains have been getting worse My Specialist put a ballon down the oseghegus/Gullet to widen the scare tissue…but that did nothing……No much pain though…But I am still having swallowing trouble and just last few weeks reflux has started again with choking….

    • Hi Graham, I am really sad to know your experience with POEM. I had my POEM in Oct 2015 and doing quite well till now. I had only taken the medicine for 1ft month only, after that till now i am not having any issues with it.

As far as i researched the POEM Procedure is the safest one with great durability. The First Patient with POEM Surgery was done by Dr Innou in 2008 and till now that Patient is in monitoed for any sideeffects of POEM. They Didnt find any… On the Track Records of POEM I ad also gone through it and I can Says its Miracle for Me.

Any way You can Consult your Doc in NZ or I could suggest to Come to India . Specially Dr Amol Bapey who is the Pioneer in POEM and i was his 67th Patient Last year,

      Wish you best of Health

      • Hi There saumendra 007

Thank you for that message…..Just recentary I have run into a very nice India nurse and she used to help with the surgery last year in India……and she told me the same thing…..Our specialist brought a Dr Froccum out from Amsterdam to operate on 6 of us…….The 1st and last there is trouble but I believe that has been fixed and I was the last and 4.15 hours operation it did not fix it….My specialist is in Holland at present to find out why….Also my Operation was filmed with my consent so I am hoping they will be able to fix it
        Stay safe
        Graham

      • Perhaps my operation did not go well because of a dead esophagus because it took 2.5 years of reflux to be diagnosed but my specialist will find out

      • Hi Saumendra

Good to hear that you are leading a normal life now.

Well I am fighting daily with this terrible disorder. I am from Delhi and i am not able to take it as how to deal it further.

Brother, request you to please share your number or email id at ankush7251@yahoo.co.in so that i can get in touch with you to discuss more on POEM and the place to get it done.

May be by your help and guidance one more life could be saved.

        Regards
        Ansh
